The OFW Privilege Card is a gift from American Eye Center to all OFWs and their qualified dependents. This is American Eye Center's way of recognizing the OFWs as modern day heroes. How much is the card? There is no application fee. The card is free to all OFWs and their dependents. Who are the qualified dependents? Married OFWs: Spouse and children Single OFWs: their parents How long is the card's validity? The American Eye Center OFW Privilege card is valid until January 31, 2015 subject to renewal of the American Eye Center. I'm an OFW but I don't have the documents required for application, can I still apply? Unfortunately no. All applicants must first submit the requirements before claiming the card. Present any of the following valid proofs of overseas employment at any American Eye Center clinic:
If the patient is a dependent of the OFW, he or she must present the proof of overseas employment of his or her relative as listed above, along with his or her personal valid ID (SSS, school, driver's license, voter's ID, passport or birth certificate). Do the dependents have to be physically present when applying for the American Eye Center OFW Privilege card? Yes. They must submit the requirements in person to get the card. Is there an age requirement for the American Eye Center OFW Privilege? None. The card is available to all OFWs and their dependents. My OFW relative is still abroad, can I apply for him/her? Yes, the dependent can apply in the OFWs behalf by submitting the necessary requirements. Application forms are available online as well. |
